You may use the following form to schedule your next visit to HANOVER VETERINARY HOSPITAL. Your request will be submitted to our front desk and you will receive a confirmation e-mail within 24 hours. Please make sure you provide us with a valid e-mail address to send you a confirmation e-mail. DO NOT use this form if you have an urgent medical matter. In the event of an emergency, call our office immediately at 219-374-9443.
Requests received after 9am on Saturday will be confirmed on Monday. Please note: we are closed all Sundays and major holidays. At this time we cannot take boarding reservations or surgery appointments online. These may be sceduled by calling our office during our regular business hours.
